What do I need to complete while I am on shift for my clinical rotations?

  1. A PCR for every patient encounter. You can do the PCR on paper or directly in FISDAP. (If you choose to do the PCRs on paper, know that you are just using that as a template and will have to enter the patient information into FISDAP at a later time. You cannot just attach the paper version to the shift as an attachment. PCRs do not require a preceptor signature.)
  2. A clinical site evaluation for each shift attended. Even if the clinical site is the same for each shift. You can do this evaluation on paper or directly in FISDAP. (If you want to do it directly in FISDAP just click on the blue field shift created for you and then scroll down to "shift evaluations.") If you do it on paper, attaching it to the appropriate FISDAP shift is acceptable.
  3.  A preceptor evaluation for each shift attended. Even if the preceptor is the same for each shift. You can do this evaluation on paper or directly in FISDAP. (If you want to do it directly in FISDAP just click on the blue field shift created for you and then scroll down to "shift evaluations.") If you do it on paper, attaching it to the appropriate FISDAP shift is acceptable.
  4. A preceptor's evaluation of the student for each shift attended. This is the only evaluation that requires a signature from the preceptor. You should do this evaluation on paper and attach it to the appropriate FISDAP shift.
