What are the required doucments for each class?

The American Heart Association requires documentation for each conducted class. Affiliates and Training Sites within RC Health Services' Training Center are required to submit the following documents with each class roster.

Note: If any student requires remediation, RC Health Services requires all skills sheets to be uploaded, including the successful remediation as well as the failed attempt(s).

BLS Provider (Classroom):
  1. Adult CPR Skill Sheet per student.
  2. Infant CPR Skill Sheet per student.
  3. Graded Exam Sheet per student.
BLS Provider (Heartcode)
  1. Adult CPR Skill Sheet per student.
  2. Infant CPR Skill Sheet per student.
  3. Heartcode Certificate per student.
Heartsaver CPR/AED/First Aid (Classroom)
Note: Heartsaver is a modular course. You are able to mix and match which disciplines are taught, but all skill sheets must be turned in for those skills or cards will not be issued.
  1. Adult CPR Skill Sheet per student (if tested).
  2. Child CPR Skill Sheet per student (if tested).
  3. Infant CPR Skill Sheet per student (if tested).
  4. First Aid Skills Testing Sheet per student (if tested).
ACLS (Classroom)
  1. Completed Precourse Self Assessment per student.
  2. Adult High Quality BLS Skill Sheet per student.
  3. Airway Management Skill Sheet per student.
  4. Megacode Testing Skill Sheet per student.
  5. Graded Exam Sheet per student.
ACLS (Heartcode)
  1. Adult High Quality BLS Skill Sheet per student.
  2. Airway Management Skill Sheet per student.
  3. Megacode Testing Skill Sheet per student.
  4. Heartcode Certificate per student.
PALS (Classroom)
